In the course of using ZazaMD and engaging in the Service provided by your medical provider we may collect personal information that can identify you such as your name, contact information, or other identifying information (“Personally Identifiable Information”) and medical information such as medical history and current medical problems or conditions (“Medical Information”). We may collect this information through various forms and in various places and providing the Service, including through JotForm, through “contact us” forms, or when you otherwise interact with the Service in any way, such as through completing our online questionnaire and engaging in a video or phone conference consultation with your medical provider. Personally Identifiable Information and Medical Information are collectively referred to as “Personal Information.”

CMIA establishes standards to protect Medical Information of the individuals that use the Service. The user is responsible for maintaining the truthfulness and accuracy of the information you submit to us, such as the users contact information, and information provided to the medical provider in the online questionnaire and during the video or phone conference we will disable a user’s account and no longer proceed with our services if the information provided is determined to be false are no longer current.

We may automatically collect certain types of usage information when you visit our website or use our Service. When you visit the Service, we may send one or more cookies – a small text file containing a string of alpha numeric characters – to your computer that uniquely identifies your browser and lets us help you enhance your navigation through the site. We support and comply with the Children’s Online Privacy Protection Act (COPPA) and we do not knowingly collect information from children under the age of 13, nor do we share such information with third parties children under the age of 13 may not use ZazaMD.
